He is survived by: his wife Betty Sullivan Cook; his son Bradford Todd Cook; and his sisters, Ella Boldin (Oliver), Bubbis Ruth Cook and Ann Widner (Randy). He is also survived by several nieces, nephews and other relatives and friends.
