He is survived by: his wife Debbie Ray Sullivan of Milan; his sons, Guy Sullivan III (Tamela) of Lavergne, TN and Daniel Sullivan (Ashley) of Clarksville, TN.; his siblings, Chuck Humphrey (Beverly), Jack Sullivan (Kathy) and Sue Brown (Travis); and his grandchildren, Jackson Sullivan, Alex Coffin, Deklin Sullivan and Alessandra Sullivan.
