He was predeceased by: his son Donnie Cook; his parents, Hobart Cook and Mossie Cook; and his siblings, Carlyle Cook, Carolyn Norris and Oval Cox.

He is survived by: his wife Janice Irick Cook; his sons, Joey Cook (Beth) and Tim Cook (Leanne); his grandchildren, Lydia Cook, Keith Cook (Kayla) and Felicia Cook; his great grandchildren, Brianna Cook and Madeline Cook; his siblings, Charles Cook, Ted Cook (Joan), Opal Cox, Estelle Caldwell, Wanda Perry, Christine Kiser and Nell Southerland (Roe); and his sisters-in-law, Faye Cook, Myra Beeler and Linda Cox. He is also survived by Several nieces, nephews and other relatives along with a host of friends.
