He is survived by: his children, Jordan, Mark, Dewayne and Mady Curtis; his grandchildren, Wesley Crowder (Stephanie), April, Abby Crowder and Jack Cook; his sisters, Marsha Sipes (Mark) and Maureen Fredrick; the mother of his children Jeanette Curtis; his stepchildren, Jonie Crowder (Aaron) and Jerry Cook Jr. (Brandi); and his pet Jed.
