He was predeceased by: his parents, Paul Fredrick Blake and Mary Lou McDurmon Blake; and his brothers, Paul Dennis Blake and Burt Leroy Blake. He is survived by: his wife Linda Rose Morris Blake; his children, David Richard Blake (Karissa) of Martin, Dreama Blake Dolan (Brent) of Bruceton, TN and Amanda Beth McDearmon (John) of Auburntown, TN; and his grandchildren, Zachary Blake Pyles, Kaitlin Montanna Holley (Kyle), Landon Lee McDearmon, Luke Michael McDearmon and Beau Jarod Blake.
