He was predeceased by: his mother Dianne Ferguson Mahaney; his brother Sidney Chadwick Azbell; and his nephew Matthew Abel Hargrove. He is survived by: his wife Dawn; his children, Brianna Azbell, Erin Johnson (Heath), Ashley Gass (Cody), Justin Durm (Maddy) and Erica Durm; his grandchildren, Libby, Emmitt, Lainey, Brewer, Korrie Beth, Abel and Sloane; his father Billy Azbell (Karen); his sister Tonya Cochran (Barry); his nephew Michael Hargrove; his sister Nikki Hammond (Jimmy); his nieces and nephews, Caroline and Tripp Hammond; his parents-in-law, Bob and Sandy Lindeman; his sister-in-law Stacy Lynch (Daniel); and his nieces, Abby and Maggie Lynch.
In lieu of flowers, the family requests donations be made to the American Heart Association or the American Diabetes Association.
