He was predeceased by: his parents, Jerry C. Crook and Roberta Holbrook Crook; and his son-in-law Greg Widner. He is survived by: his wife Nancy Henson Crook; his son Dr. Jay Crook; his daughter-in-law Robin Crook of Knoxville; his daughter Dr. Joy Crook of Nolensville, TN; and his grandchildren, Caroline, Cooper, Ethan Crook, Evan, Emma, Jackson and Jacob Widner.

In lieu of flowers, Dr. Crook's family requests consideration of a donation in his memory to St. Jude Children's Hospital, of which Dr. Crook was a passionate supporter.
