He was predeceased by: his parents, Willie Alton and Lynn Owens Hazlett; and his sister Ann Lee. He is survived by: his children, Brian Hazlett, Alonzo Tracy, Tara Hazlett and Jennifer Hazlett; and his sisters, Donna Poarch (Andy), Diane Hazlett and Debbie Blalock (Kevin). He is also survived by nine grandchildren; two great grandchildren and his many friends.
