He was predeceased by: his parents, Charles Maddox and Carolyn Maddox; and his stepson Shane Hendrix. He is survived by: his wife Kimberly; his children, Brandon Maddox, Blake Maddox (Michelle), Adam Maddox, Davey Maddox, Kristen Bryda (Dan), Katy Leverette (Tyler), Katherine Jones (Josh), Alexis Shook (Jonathan) and Harley Maddox; his brothers, Tim Maddox (Tina) and Jay Maddox (Isa); and his son Jackson Holley. He is also survived by his loving family and friends.
In lieu of flowers, the family requests that donations be made to a trust for his youngest daughter, Harley. Donations can be made to CB&S Bank directly to the account of Harley Maddox.
