He was predeceased by: his parents, Otha Keith, Sr. (Tobe) and Mary Shipman Smith; and his siblings, W. C., Melvin (Pill), Otha Jr. (Tootsie) and Ruth Hanback.
He is survived by: his wife Fana Berry Smith; his sons, David Smith Jr. of Lawrenceburg, TN and Keith Smith (Aimee) of Hendersonville, TN; his grandchildren, Will, Zach and Mary Smith; and his sister Margaret Daniel. He is also survived by many nieces and nephews.
