He was predeceased by: his mother Jan Kelly. He is survived by: his wife Chris Brooks; his daughters, Karri Brooks and Jessica Brooks Hamrin; and his grandchildren, Tyler, Kaden, Phoenix, Kylee, Declan and Jensen.

In lieu of flowers donations may be made to Cross-Smith Funeral Home for funeral expenses.
