She was predeceased by: her parents, William Sampson and Flossie Walden McCullah; her grandchildren, Christopher Sampson Ayers and Jennifer Renee Chadwell; her son-in-law Barry Forsyth; her great grandchild Angelina; her siblings, Azalee Allen, Verna Jordan, Mary Ellen Franklin, George Andrew McCullah and Hubert Monroe McCullah; her close friend Rev. Bill Loy; and also Atlas.
She is survived by: her daughters, Sheila Sheckles (Danny), Joyce Reese (Robert) and Felicia Wilson (Tom); her grandchildren, Travis Forsyth (Carrie), Joseph Ayers (Tara), Matthew Forsyth (Carlie), Kelsey Simms (Nicholas), Dakota Anderson (Jimmy), Kaitlynn Baird and Holly Crabtree (David); her great grandchildren, Aiden, Katie, Raven, Eddie, Jocelyn, Kinsley, Kamden, Hannah Beth, Taylor and Kollins; her great great grandchild Elliegh; her great grandchildren, J. T. and Sabrina; and her siblings, Zeta Kahn, Charlotte Robbins, LoVada Kitts (Wayne) and Rev. Jerome McCullah (Sylvia). She is also survived by Numerous Nieces, Nephews, other relatives and friends.
