He is survived by: his wife Orilla Dawes Cook; his children, Charles Cook and Joyce Agee (Homer); his stepsister R. C. Graham; his grandsons, Dewayne Cook (Michelle Shelley Birdwell), Tim Kim Carnahan Corinne and Brian Chad Carnahan Lindsey; his great great grandchild Host; and also Survive. He is also survived by Eleven Great-Grandchildren.

Family requests donations be made towards Days Cross Roads Missionary Baptist Church.
