He was predeceased by: his parents, Hugh Pascal Slack and Elizabeth Record Slack; and his siblings, Danny Slack, Betty Groves, Eva Charleston, Sara Jane Alford and Valerie Slack.
He is survived by: his daughter Amanda Couch (Daniel) of Lafayette; his sister Mary Ann Slack of Gallatin; his grandchildren, Destiny Groves, Brayden Groves (Sophia) and Ethen Groves; and his great grandchildren, Carson Groves, Colten Groves, Trevor Groves, Paisley Groves and Mya Groves. He is also survived by several nieces and nephews.
