He was predeceased by: his parents, Jerry Collingsworth and Shirley McCray. He is survived by: his wife Kate; his children, Josh, Javin, Alexis, Jaxson and Jayce; his sister Gail Collingsworth; his son-in-law Dustin Gordon; his in-laws, Bre and Adam Calderoni; his nieces, Luna and Lily; and his father-in-law Clay Clement. He is also survived by several aunts and uncles and an excessive number of cousins.
His last generous gesture was donating his organs to help others.
