He was predeceased by: his parents, Fred Lee and Dorothy "Dot" Jean Sullivan Jr.. He is survived by: his wife Cheryl Lynn Sullivan; his brother Tom Sullivan; his daughter Ashley Sullivan Crook (Jayce); his grandson Jayden Crook; his stepson Chad Moore; and his stepgrandchildren, Dylan Moore and Cheyenne Moore.

“I have known Frederick T Sullivan since Jr. High School. He has been not only my hero but also my best friend. I looked up to him in every thing he attempted. We ran around together after high school and did until he married the first time. He then moved away and picked up the banjo and was a master of the instrument as well as the guitar. That also made me to idolize him. He seemed to enjoy life to every extent. I pray that he will rest in peace after going through such a trying ordeal.”
