He was predeceased by: his father Richard Slate. He is survived by: his stepfather Bruce Armstrong of Hermitage; his brothers, Kevin Slate of LaVergne and Chris Slate of Lafayette; his nephew Justin Slate; and his nieces, Amber Manning, Kayle Slate and Kelsie Slate. He is also survived by aunts, uncles, and cousins.
