He was predeceased by: his parents, Ed Cook and Ruthie Blanche Cook; his brother Robert W. "Billy" Cook; and his stepson Joey Rutter. He is survived by: his wife Emma Kate Cook; his sons, Steve Cook (Monica), Michael Rutter (Elizabeth) and Jeffrey Rutter (Ann); his sister Peggy Mires (Lon); and his sister-in-law Jane Cook. He is also survived by eight grandchildren, 3 great grandchildren.

In Lieu of flowers, the family requests donations be made in Jerry's name to the Alzheimer's Association Tennessee Chapter.
