He was predeceased by: his parents, Lethia Sylvis and James C. Sylvis. He is survived by: his wife Carol; his daughter Shannon Haylett (Todd); and his granddaughters, Ashley, Amanda and Alivia. He is also survived by "Papa" of his daughter and three granddaughters.
