He is survived by: his wife Arvelia Hayes; his daughters, Terry Shahrivar (Shahryar), Deborah Hayes and Melissa Hayes; his grandchildren, Phillip Shahrivar (Courtney Madsen) and Shireen Shahrivar (Keith Bolling); and his brother James "Gabby" Hayes (Linda).
