He is survived by: his wife Sue (Huls); his children, Jeffrey Forman (Karan) and Susan Bennet (John); his sister Carol Clayton (Howard); his sister-in-law Chris Huls; and his grandchildren, Mallory Fiery (Michael), Stacey Ater (Nick), Ben Forman (Lauren), Bethany Bennet, Shawn Andrews (Amy), Chad Andrews (Lisa), Lindsay Koehl (Michael), Ashley Gray (Tony), Whitney DuBois (Bill) and Tim Bennet (Rachael).
