She was predeceased by: her grandparents, Clarence Johnson, Mary Ruth Johnson, Damon Sergerson and Cecelia Sergerson; and her aunt Marsha Arnold Phillips.
She is survived by: her husband Gary Cook; her sons, Jonathan Ryan Cook and Colin Reece Cook; her siblings, David B. Sergerson, Dawn Black and Deana Mangold (Rick); and her parents, David Sergerson and Freida Sergerson. She is also survived by nieces, nephews, aunts, uncles, cousins, many extended family and friends and last but not least, Smokey the cat.
In lieu of flowers, donations in Dayna's name can be made to the Sarah Cannon Cancer Center, 2300 Patterson St, Nashville, TN 37203 or to St. Jude Children's Research Hospital.
