She is survived by: her husband Jerry Polk; her children, Stacey Bowers, Tracey Cook (Jeff), Christy Polk and Drew Polk; her grandchildren, Tyler, Landon, Brianna, Jaedyn and Joseph; her great grandchild Eli; her siblings, Becky Stewart, Mike Evans and Jennifer Hill; and her close friend Susan Huff.
