He was predeceased by: his son Gary DeWayne Perkins; his parent Shirley Perkins; and his siblings, Faye Philyaw and Rodney Perkins. He is survived by: his wife Rita; his sons, Samuel Perkins (Angel) and Bradley Perkins (Amy); his granddaughter Bailee Statzer; and his siblings, Wanda Frye, Deloris Shelton, Dexter Perkins (Mandy) and Joe Perkins.
