He was predeceased by: his parents, W. F. Phillips and Hester Poore Phillips; and his siblings, Stewart Phillips, Harley Phillips, Cecil Phillips, Dollie Wright and Helen Bailey.
He is survived by: his wife Alma Jean Woodby Phillips; his daughters, Terrie Maddox (Guy Maddox Jr.) and Cheryl Buchanan; his grandchildren, Kelly Maddox, Marcus Maddox, Shane Forkin (Kim) and Kris Forkin (Khrystyne); his great grandchild Bryce Forkin; and his stepgreat-grandchildren, Addie and Ashton.
