She was predeceased by: her parents, Ernest Cook and Wilma Cook. She is survived by: her husband Ed Eubank of Greenfield; her children, Eddy Eubank, Eve Richardson, Abby Eubank and Mercee Deforge; and her sisters, Brenda Hazelwood, Barbara Hazelwood and Debbie Asher. She is also survived by seven grandchildren and one great grandchild.
