He was predeceased by: his parents, Tom Blake and Malcena Dixon Blake; and his brother Kenneth Blake. He is survived by: his children, Amanda Foulks (Greg), Rev. Jeff Blake (Teri) and Lionel Blake; his grandson Scott Blake (Jasmine); his great grandchildren, Carter Blake and Cash Blake; his sisters, Dixie Blake and Madge Walker (Vance); his sister-in-law Marie Blake; and his nephews, Chris Blake and Freddy Blake.
