He was predeceased by: his wife Mary Louise Ralph; his parents, Tyree W. Ralph, Sr. and Ozie Hall Ralph; and his siblings, Betty Sue Black, Margie Christine Goats, Mary Lee Brown, T. W. Ralph, Jr. and Don Henry.

He is survived by: his sister Doris Hall Dean (Bill Dean); his daughters, Luwana Ralph and Barbara Ralph Kizer (Millard W. Kizer, Jr.); and his grandsons, Millard Kizer, III, David Wayne Kizer (Alisha Kizer) and James Michael Kizer. He is also survived by thirteen great-grandchildren, one great-great granddaughter, numerous nieces and nephews.
