He was predeceased by: his parents, Corinne J. Cook and Carlisle F. Cook, Sr.; and his wife Juanita Carole Ross of Philip's Church. He is survived by: his sons, Ken Cook (Pauline) and Terry Cook (Laura); and his brother Robert Cook (Susie). He is also survived by seven grandchildren and seven great-grandchildren.
