He was predeceased by: his father Calvin Howard Phelps. He is survived by: his wife Susan Phelps; his children, Christopher Phelps, Bobby Phelps and Katie Phelps; and his siblings, Howard Phelps (Joyce), John Phelps (Joanne) and Leslie Perlik (Paul Perlik).
