He was predeceased by: his parents, Curtis Jack and Juanita Jack; his brother Ronnie "Butch"; his sister-in-law Norma Jean Jack; and his grandparents, Frank, Dellie Jack, Leo and Mae Tallent.
He is survived by: his wife Joyce Jack; his sister Rebecca Sue Raby (Paul); his nephews, Stuart "Topper" Raby (Lisa), Adam Raby (Eva) and Donald Jack; and his aunts, Ruth Crye (Elmer) and Etoile Carter. He is also survived by several cousins, great-nieces and nephews.
