He is survived by: his wife Teresa Wilson Forshay; his daughters, Laurie Farrow (Todd) and LeeAnn Gilbert (Dale); and his brother Mickey Forshay (Connie). He is also survived by 6 grandchildren; 4 great grandchildren.
