He was predeceased by: his parents, James Alex and Julia White Koellein; and his former wife Suzanne Hotchkiss Koellein. He is survived by: his wife Sandra Ray Fuller Koellein of Dickson, TN; his sons, Mayhew Franklin Koellein, Jr. (Linda) of Dickson, TN and James Alex Koellein II (Mary) of Adams, TN; his stepchildren, Sherry Jeannine Allen (Bill) of California and Richard Fuller of California; his grandchildren, Elizabeth Russell (Jeremy), Evan, Tate and Emily Koellein; and his stepgrandsons, Tanner and Trent Allen.

In lieu of flowers memorials may be made to the American Cancer Society or the Charity of your choice.
