He was predeceased by: his parents, Grover Cleveland and Ima Kate Dennis Polk; his son Richard E Polk; and his siblings, Earl, Jim, Carl, Joe Polk, Catherine Creel and Mary Marco.
He is survived by: his wife Ouida Collins Polk of Dickson, TN; his children, Denise Adams (David) of White Bluff, TN and David Polk (Leann) of Dickson, TN; his grandchildren, Heather, Griffin, Sean, Will, Ashlee and Brooklyn; and his great grandchildren, Mandilynn, Abbigrace, Eli, Olin, Mason and Chloe.
