He is survived by: his mother Brenda Kay Blaylock of Dickson, TN; his girlfriend Lacey Swinehart of Dickson, TN; his children, Luke, Keith Blaylock, Jon Dawson, Amanda, Amelia and Addison Blaylock; his siblings, Greg Blaylock (BJ) of Dickson, TN, Jason McLellan (Heidi) of Lapeer, MI. and Tonya Roberts of Dickson, TN; and his grandchildren, Jude, Lily and Jettson.
