He was predeceased by: his parents, Dyke Ballard and Thelma Ballard (Wilkey); his wife Evelyn Ballard (Caraway); and his siblings, Wallace "John" Ballard and Pat Francisco.
He is survived by: his children, Larry Ballard (Debbie) of Dayton, Theresa Snyder (Tommy) of Dayton and Cindi Smith; and his siblings, Bruce Ballard (Lounell) of Dayton, Larry Ballard (Evelyn) of Dayton, Tommy Ballard (Kathy) of Dayton, Linda Parker (Bobby) of Knoxville and Charlotte Dotson of Dayton. He is also survived by seven grandchildren; 23 great-grandchildren; three great-great-grandchildren.
