He was predeceased by: his parents, Lee Roy Underwood and Annie Ophelia Baggett Underwood; and his brother Jerry Underwood. He is survived by: his children, Stephen Underwood (Simone), Dawn Marie Swafford (Tony), Andrea Underwood and Racheal Forrest (Ron); his sisters, Peggy Riels (Larry) and Teresa Sinks (Eddie); and his grandchildren, Joshua Underwood, Sierra Edwards (Douglas), Zachery Sturdivant, Conner Sturdivant, Nicholas Dennis, Katelyn Dennis, Kristen Sikes (Shea), Meagan Ybarra, Tayler Forrest and Nathan Forrest. He is also survived by five great-grandchildren, five nieces and nephews, and one great-niece and three great-nephews.
