He is survived by: his wife Gail Boles; his sons, Kenneth Boles (Wendy) and Kevin Boles (Missy); his grandchildren, Tyler Boles (Allison), Cheyenne and Shashauna Boles; his great grandchild Mason Boles; his siblings, Gene Boles, Della Goodman and Patsy; and his close friends, David and Judy Boling.

The family request in lieu of flowers donation be made directly to Mynatt Funeral Home in honor of Herb.
