He was predeceased by: his mother Hettie Hammons. He is survived by: his wife Janet Cunningham Qualls; his sons, C. H. Qualls (Kristin) and Houston Qualls; his grandchildren, Raley, Karsyn, Jackson, Brady and Natalie Qualls; his brother-in-law Danny Cunningham (Melody); and his nephews, Daniel and Travis Cunningham.

In lieu of flowers, the family requests that a memorial contribution be made to the "Fairview Baptist Church Standing on the Promises Building Fund".
