He was predeceased by: his father David Smith, Sr.. He is survived by: his mother Mary Jo Blair Chapman (Henry); his daughter Jastyn; the mother of his child Martina Smith-Bolanos of Smyrna, TN; his stepchildren, Jaclissa Haislip of Atlanta, GA and Drew McLean of Vero Beach, FL; and his siblings, David Smith, Jr. (Tyree) of Lewisburg, TN, Marc Smith (Tracy) of Hickory, NC and Kristen Smith of Lewisburg. He is also survived by numerous nieces and nephews.
