He was predeceased by: his father Larry Hazelwood; and his grandfathers, J. W. Hazelwood and John W. Hood. He is survived by: his children, Johnna Hazelwood and Will Hazelwood; his mother Donna Hazelwood; his siblings, Cathey Polk (Bryon) and Jamie; his nieces and nephews, Grant, Brett Polk, Atlee and Josie Hancock; his grandmothers, Mattie Sue Hazelwood and Elene Hood-Butts; and his close friend Doreen Jackson. He is also survived by many aunts, uncles and cousins.
