She is survived by: her brothers, Clayton and Ralph Cook; her husband Alvin Forsythe; her sons, Doug Farmer (Jane), Charles "Bub" Farmer, Eugene Farmer (Angie) and Of Clinton; her stepson Alvin Forsythe Jr. (Shelley) of Ky; her grandchildren, Tiffany Lively (Justin), Mason Farmer, Charley Grace Farmer, Ryan Forsythe and BJ Forsythe (Emma); her great grandchildren, Eli Lively and Ali Lively; her sister Brenda Carter of Harrison; her sister-in-law Karon Cook of Delphi; and her close friend Mildred Lively.
