He was predeceased by: his parents, Shirly and Travis Forsythe Sr.. He is survived by: his girlfriend Tina Edwards; his brother Roy Forsythe of Marlow; and his nephews, Justin, Jeremy and Jesse. He is also survived by extended family and special friends.
