He is survived by: his wife Regina; his sons, Nicholas (Tiffany), Jonathan and Luke; his grandsons, Andrew and Matthew; and his siblings, Marie Azzara Sass (Stephen), Joe Azzara, Ernest Azzara, Timothy Azzara, Jean Azzara Pirrotta (Rick), Agnes Azzara Brooks (Steven), Margaret Bannan, Alyssa Azzara, Veronica Azzara and Patricia Azzara Samuelian (Frank).
