He was predeceased by: his father Louis Millard Chadwick; and his brother Louis Millard Chadwick Jr.. He is survived by: his wife Mildred Chadwick; his mother Mary Chadwick; his daughter Lisa Bales (Dewayne); his sisters, Debbie Chadwick and Barbara Teague; his grandchildren, Brianna Ainsleigh, Taygen, Triton, Talan, Jase, Cole and Abby Shadden; and his niece Adrienne Moyer (Chris).
