She is survived by: her children, Kathy Forman of Deerfield Beach, FL, Barbara Simmons of Boca Raton, FL, Ann Key of Cookeville, TN, Brian McAfee of Lake Worth, FL, Brad McAfee of Delray Beach, FL and Eddie Bowen of Cordele, GA; and her brothers, Bill Howard of Kuttawa, KY and Lewis Howard of Lewisburg, TN. She is also survived by several grandchildren and great grandchildren.
