He was predeceased by: his parents, Charles Edward "Charlie" Forsythe and Pauline Adams Forsythe. He is survived by: his children, Kim Dodge (Monty) of Columbia, MO, Chris Forsythe (April) of Murray, KY and Andy Forsythe (Kelly) of Greenfield, TN; his sister Martha Jo Houston of Paris, TN; his grandchildren, Amanda DuBois, Blake Cox, Houston Forsythe (Megan), Zach Forsythe (Layla), Sayde Hindelang, Connor Kelley, Taylor Hindelang, Nick Brown and Layton Brown; and his uncle Jerry Forsythe (Venie) of Henderson, TN.. He is also survived by three great grandchildren.
