He was predeceased by: his parents, Joyce Puckett, and Clayton Puckett, Sr.. He is survived by: his daughters, Ashley Puckett and Trista White; his grandchildren, Makenzi Davis (Anthony), Nathan Morrison and Jensen Puckett; his great grandchildren, Jayce, Oliver, Alex and Boogie; his siblings, Pamela Bennett and Mark Puckett; his nephew Shaun Correll; his niece Sarah Puckett; and his great-niece Haley Correll.

In lieu of flowers, donations may be made to St. Judes.
