He was predeceased by: his parents, Marvin Cook and Maxine Cook; and his brother Larry Cook. He is survived by: his wife Susan Cook; his children, Michael Cook (Veronica) of Collierville, Ryan Cook (Lindsey) of Atoka and Jessica Wyatt (Jamie) of Atoka; his stepdaughters, Lisa Burke (Danny) of Roswell, NM and Amy Cantu (Arnold) of Greeley, CO; and his brothers, Jerry Cook of Martin, Danny Cook of Martin and Edward Cook of Munford. He is also survived by 11 grandchildren; three step-grandchildren and two step-great grandchildren.
In lieu of flowers, the family requests that memorials be made to the American Liver Foundation.
