He is survived by: his wife Nancy Sladek; his children, Jennifer Hyams (Todd), Kristen Coller and Thomas, Jr. (Katie); his grandchildren, Phillip, Kristen Hyams, Stella, Fred Coller, Thomas III, Cade and Gage Sladek; his brother Michael Sladek (Deborah); his nieces, Ember Fry (John) and Chava; and his nephew Frank Sladek (Jelena).
